PHARM 1095 - COMMUNITY CONNECTOR COURSEMinimum Credits: 2 Maximum Credits: 2 This seminar focuses on the patient population which is a high risk population who are vulnerable to frequent re-admissions to the hospital. Emphasis will be placed equally on 1) understanding this patient population, the health system and health behavior challenges associated with this population, and systematic needs associated with reducing their health care needs and 2) exposing students to the patient population through visits at the hospital, home, and community including providing in-home assessments, patient education, and behavior change techniques. The course will include presentations by researchers, clinicians, and industry experts involved in these areas associated with this patient population. Students will be paired with a patient and follow them from the hospital to their home. Academic Career: Undergraduate Course Component: Lecture Grade Component: Letter Grade
